2012 Health Screenings
July 1 through September 30

$100 for each target you meet
(up to $400 if you meet
all four targets)
$50 improvement incentive for
any target you missed last year
but meet this year
(up to a maximum of $100)
Your enrolled spouse or domestic partner can earn the same incentives. Together, you can earn at least $800 if you both meet all targets – and even more if you meet any for the first time.
All associates and enrolled spouses/domestic partners must get a health screening between 7/1/2012 and 9/30/2012 to qualify for any incentives and have a choice of all Kroger medical plans for 2013. There is one exception: Anyone who had a screening in 2011 and met all four targets is not required to get a screening this year (but must still complete the HQ by the deadline).
Kroger pays the full cost of health screenings for you and your spouse/domestic partner between 7/1/2012 and 9/30/2012. If you go to the doctor, be sure the visit is coded as preventive care.
You and your spouse or domestic partner must individually register at yourkrogerbenefits.com before your confidential results can be uploaded to qualify for incentive dollars.
If you or your enrolled spouse/domestic partner has a medical condition that causes any screenings to be above target, you may still be eligible for the incentive. Click here to email the Reasonable Accomodation Form to your doctor. Have your doctor fax the completed form to the Kroger Benefits Service Center at 503-797-3793. You can also download and print the form from yourkrogerbenefits.com.
